


Surrogate Eligibility Requirements
Qualifications to become a gestational surrogate
To ensure a safe and successful surrogacy journey for both the gestational surrogate and intended parents, candidates must meet certain medical, lifestyle, and legal requirements. While final approval is determined by a fertility clinic, the following guidelines reflect current industry standards.
Basic Surrogate Requirements

Between 21–42 years old
(Some clinics may allow up to 43–45 depending on health and history) -
At least one prior healthy, full-term pregnancy and delivery
(No major complications) -
Currently raising at least one child

U.S. citizen or legal resident

Body Mass Index (BMI) typically under 32–35
(Varies slightly by clinic; most prefer under 32 for IVF success)
Health & Lifestyle Requirements

In good physical and reproductive health

Non-smoker (including vaping)

No history of drug or substance abuse

Limited or moderate alcohol use

Not currently taking medications that would interfere with pregnancy
Stability & Support

Stable home environment and support system

Reliable transportation for medical appointments

Ability to attend regular monitoring visits

Willingness to follow medical protocols and take prescribed medications
Additional Considerations

No more than a limited number of prior C-sections (typically 2–3, depending on clinic)

Willingness to complete medical, psychological, and legal screening

Comfortable with the emotional and physical commitment of pregnancy for another family
Screening Process
These requirements are in place to help ensure the health and well-being of everyone involved — especially the surrogate and baby. Each candidate is evaluated individually, and some flexibility may be considered depending on overall health and pregnancy history.
